Our practice is different from many others, so please review the below practice information thoroughly. There are additional fees for which you will be responsible, so don’t sign anything until you fully understand our practice model. We look forward to meeting you!

Integrative Therapies Financial Info

Things you need to know before making your first appointment:

1. We are currently scheduling new patients. We are booking up to 2 months out in order to allow time on the schedule for our established patients.

2. We accept the following insurances:

  • Aetna
  • Blue Cross/Blue Shield
  • Cigna
  • HPHC
  • Martins Point
  • MCHO
  • Medicare
  • UHC

3. We accept self-pay. The rate is 165.00 per hour (membership fees still apply for those seeking assistance by e-mail and phone for ongoing health concerns outside of office visits, alternatively that time can be billed to you with a card on file).

* We are also credentialed with: HPHC, Cigna, and Anthem healthcare exchange care plans (Obamacare plans).

If you are uncertain as to whether or not we take your insurance please verify with our office PRIOR to scheduling your new patient appointment. If we do not take your insurance and you schedule an appointment you will be charged the cash price for your visit.

To use your insurance in our practice we require an annual fee – please CAREFULLY read #5 below to ensure you understand what this means.

4. We have fewer support staff than a conventional medical practice.
We have chosen to provide long appointments and high quality integrative care. Consequently, longer appointments require a low overhead model. One way we do this is by having fewer support staff.

WHAT DOES THIS MEAN FOR YOU:
This means that you have to be active in your healthcare. Patients are required to pay all invoices promptly to continue seeing their provider. All invoices are sent to your email with an explanation of what is being charged and for what visit.

5. We charge all health insurance participants an Annual Fee, beyond your insurance premium, deductible, copay and coinsurance.
This fee, $25/mo. or $300/yr. for individuals, $50/mo. or $600/yr. for families (2 or more people), covers all the things we do for you not covered by your insurance. It is a membership fee and holds your place in your provider’s panel. It keeps the practice sustainable and allows us to continue taking insurance and have long appointments.
* BC/BS primary care patients have the option of opting out of the membership fee if they are not seeking functional health care or concierge services and are instead looking for standard allopathic medical care without the concierge services provided at Integrative Therapies. Please discuss this with us at the time of making your first appointment.

WHAT DOES THIS MEAN FOR YOU:
You will be required to set up automatic annual payments via credit or debit card (paying monthly is also available) each year you are a patient in the practice. The Annual Fee is in addition to your insurance premium, deductible, coinsurance and copay. This means to be a patient in our practice and to have us bill your insurance you are required to pay the following –

  • Your insurance premium – what you (or your employer) pay each month for health coverage.
  • Your insurance deductible – the amount you are required to pay before your insurance will cover any portion of your healthcare costs.
  • Your insurance coinsurance – the percentage of the remaining costs you must pay after you reach your deductible
  • Your insurance copay – a flat fee that you pay at each doctors visit depending on your individual plan.
  • Your Annual Membership Fee – a membership fee charged by our practice. This fee is not refundable

Our office will send you an invoice to your email for all services not paid for at the time of your visit. We will then charge your card on file. We do NOT send invoices via snail mail. If you need a hard copy of your bill you can print the invoice from your email. As mentioned above, the invoice will note what the charge is for and for which visit to our office. If you have questions regarding your health coverage it is best to call your insurance company. The support number can be found on the back of your card. Additionally, you will receive an EOB (Explanation of Benefits) from your insurance company prior to being billed by our office. This EOB explains what your insurance paid and did not pay based on your coverage.

6. Computer access is necessary to participate in our practice.
New patient paperwork is delivered electronically, appointments can be scheduled and secure messaging accomplished through our online Patient Portal. Secure Messaging is the primary and preferred form of communication.

WHAT DOES THIS MEAN FOR YOU:
If you do not have internet access, then you may find our practice challenging.

7. Our practice requires a partnership between the practice and the patient.
We promise to do our best in delivering quality healthcare to each and every patient. In return we ask that you understand the role you play in your healthcare.

WHAT DOES THIS MEAN FOR YOU:

  • Scheduling and following through on recommended lab tests
  • Complying with detailed health protocols designed by your provider
  • Refilling your supplements as needed
  • Contacting your health insurance if claims remain unpaid
  • Providing prompt and timely payment of invoices for services provided

8. Regaining your health can be financially challenging for some patients.
Your health insurance will cover a portion of the services we offer. Depending on your health needs you will likely require products and services that are not covered by your insurance plan. Before joining our practice please understand what your insurance covers and what it does not cover.

WHAT DOES THIS MEAN FOR YOU:
Additional funds may need to be budgeted for labs not covered by insurance or for the cost of supplements. That amount will vary based upon your individual situation.

Your insurance may also require a copay for each office visit and coinsurance. We do not specialize in the details of each and every plan so please contact your insurance company to learn more.

When needed, we can work with a smaller budget for supplements and labs.

Please Note: We do not charge interest, therefore, we are unable to offer in-house financing or payment plans. If you are unable to pay for your services in full with cash or money order, you may put the balance on your credit card and make monthly payments to your credit card. Failure to pay any money owed to the practice within 30 days from date of invoice may result in dismissal from the practice.
